Why was Dedicated not placed on Hybrid Theory EP?

How are we supposed to know? We're not the band. But, if you ask me, I think they just wanted to keep the track in the dark for a while or probably just didn't want it to be on the EP.

Why do you keep asking why songs weren't put on albums? There's reasons they weren't. Most of them, only the band knows.
